Our staff and physicians are working with patients and families to get that important conversation started, if you haven’t already.
Advance Care Planning is a process of reflection and communication. It is a time for you to think about your values and wishes, and to let your substitute decision maker (SDM) know what kind of health and personal care you would want in the future if you were unable to speak for yourself.
It is applicable to everyone, not just those who are sick and/or elderly.
Who will make decisions for you if you become unable to do so on your own?
Having a plan in place can ease the burden on the care team, on the patient and family and on the SDM, who may have to make decisions about a patient’s care if the patient becomes unable to do so on their own.
During a hospital stay, your caregiver may ask you about your advance care plans.
